Patient Policies

Insurance Affiliations

Urology Health Solutions and Dr. Lotenfoe are Medicare Providers. We also accept most major medical insurances. For more detailed and current information, please ask at the office.

Payment

Payment is expected at the time of service unless other arrangements have been made in advance. All deductibles, co-pays and balances are expected to be paid at the time of the visit. If you have Medicare without supplementary insurance, your 20% co-payment is expected at the time of service. Billing may be subject to administrative service charge. We accept Visa and MasterCard. We reserve the right to refuse checks.

Privacy Policy

We respect your privacy in accordance with HIPAA. A brochure explaining our privacy policy in detail is available upon request.

Your Office Visit

We want to make your visit to our office as pleasant as possible. Therefore, we request that you follow these guidelines when visiting our offices.

  • If you are a new patient, please arrive about 15 minutes prior to your appointed time so that the administrative part of your visit can be completed in a timely manner.
  • Don’t forget to bring your insurance card with you.
  • If you are referred to Dr. Lotenfoe by another physician, by the emergency room, or if you transfer from another doctor, please bring a copy of your records including pertinent laboratory reports and X-ray reports with you to the office. Do not rely on them being mailed or faxed to our office.
  • Bring your medication bottles or a list of your current medications with you to the office.
  • Wear comfortable clothes.
  • If you have changed your insurance, contact information, address and/or name, please notify the desk on arrival.
  • If there has been a change in your medications or in your medical condition, notify the doctor or his assistant.
  • Please bear in mind that we work from an appointment schedule and coming to the office early does not necessarily imply that you will be seen early.
  • Please consider that we do our best to stay on schedule but because Dr. Lotenfoe is the only urologist at some of the hospitals, he can be called for emergencies. This may cause delays or you may be asked to reschedule.

Cancellations and Rescheduling

If you have to cancel your appointment, please call us at least one business day in advance. Our time slots are limited and not keeping your appointment deprives other patients of timely care. No-shows without proper notification are subject to a charge of $50 dollars or more. Scheduling of surgeries is a complex task that requires coordination of multiple services, is labor intensive and is time consuming.  Above all, not showing up for surgery, rescheduling without advance notification or canceling of surgery deprives other patients of much needed operating room time slots. Therefore, if you are a no-show for surgery for non-medical reasons, you will be charged $200 dollars or more.
